The credit cards we accept are MasterCard, Visa, American Express and Discover. To apply for credit terms with Office Supply Max print out a credit application and fax it to (818) 344-3822.

IMPORTANT NOTE

As one of our fraud prevention measures, make sure the billing address you input matches exactly your billing address where you receive your credit card statements. If the address you provide on this form does not match the credit card bill address your order will be delayed and possibly cancelled.

Once you fill out the form, click on the "Pay Now" button at the bottom of the page to submit your order.

We also accept Money Orders.

If you choose to select this payment type, please follow the next 4 steps for fast processing:

1) Fill in your billing and shipping information on the order form.
2) Select the "COD" option
3) Our system will generate a total and order number. Send your money order to:

* Money orders must be in US dollars drawn on a United States bank. We do not accept personal or company checks, sorry.
